Fillmore has investigated the complex interplay between systemic comorbidities, frailty, and therapeutic outcomes in patients with advanced malignancies. His research identifies how molecular signatures can stratify prostate cancer patients for precision therapy and demonstrates that specific clusters of chronic illness are more predictive of mortality in advanced non-small cell lung cancer than total comorbidity counts. Additionally, he has examined the cardiovascular risks associated with advanced cancer, finding that immunotherapy does not increase the incidence of venous thromboembolism relative to chemotherapy.
