Fillmore has investigated the systemic and pharmacological determinants of Alzheimer disease and related dementias, with a particular focus on the role of inflammation. His research demonstrates that elevated neutrophil-to-lymphocyte ratios and platelet reactivity are independent predictors of cognitive decline and correlate with amyloid and tau pathology. Additionally, he has identified potential neuroprotective effects associated with tumor necrosis factor inhibitors and SGLT2 inhibitors in high-risk populations.
