Fillmore examines the mechanisms by which clinical evidence is translated into practice, identifying the drivers and barriers that influence the adoption of medical standards. His research demonstrates that while long-term trends often align with formal guidelines, as seen in the widespread adoption of active surveillance for prostate cancer, acute clinical shifts may be driven more rapidly by preprints and non-traditional communication channels. This work highlights a critical implementation gap between the publication of evidence and the standardization of care across large healthcare systems.
